Patient enquiry forms collect your name, phone number, email address, broad dental concern and preferred appointment timing. Professional callback forms collect the dentist’s name, phone number, professional email, practice name, GDC number, broad referral area and preferred callback timing. We also record the page used and submission time so the team can route and audit the enquiry.

Please keep clinical details out of website forms

Do not enter medical history, diagnoses, patient names, radiographs or clinical records into these website forms. A broad dental concern can reveal limited health information, so Smile2Life uses it only to route the requested contact. The team will explain the appropriate secure process for any clinical information that is needed later.

How information is used and stored

Your submission is sent securely to the website service, stored in Smile2Life’s protected CRM database hosted on Hostinger so the team can respond, arrange the appropriate next step, keep an enquiry record and resolve delivery issues. Authorised practice staff can access the enquiry in the CRM. Resend delivers practice notifications and a short acknowledgement email to you. Practice notifications contain the contact information and enquiry category needed to respond. Smile2Life does not sell enquiry information or use it for marketing without separate permission.

Retention and sharing

Enquiry information is kept only for as long as it is needed to respond, manage the resulting relationship, meet applicable clinical or administrative obligations and handle disputes. Information may be shared with Hostinger for website and CRM hosting, Resend for email delivery and the practice’s email provider, or where disclosure is required by law or professional duties.

Your choices and rights

You may ask for access to, correction of or deletion of your personal information, or raise a concern about how it is used, subject to applicable legal and professional record-keeping duties.
